London-Middlesex Youth Wellness Hub

Youth Wellness Hub Ontario (YWHO) offers mental health and substance use services, primary care, and community and social services—for youth between the ages of 16 and 25. Through collaboration among many different providers and sectors, hubs provide easy and rapid access to services that address the needs of youth across multiple domains of their lives. The hubs are local places where young people can receive walk-in access to high-quality, integrated, mental health and substance use services, as well as other health, social, and employment supports.

Youth Action Centre 

The Youth Action Centre (YAC) provides a multitude of services for youth ages 16-24 who are experiencing or at risk of experiencing homelessness in London. Located downtown at 332 Richmond St, youth can access basic needs resources, meet with housing workers, and connect with community supports Monday through Friday from 10am-6pm. 

Available at the YAC: 

  • Bagged lunches served from 11am - 3pm
  • Grab & go hot dinner available from 4 - 6pm
  • Emergency food bank
  • Support with finding safe, affordable housing
  • Laundry and shower facilities
  • Hygiene products and basic needs items
  • Children's supplies, including diapers and wipes
  • Community resources

Youth-in-Transition Program

Our Youth-in-Transition program supports young people aged 16 to 24 who are leaving or connected to CAS care (youth who are a Crown Ward, participating in a Voluntary Youth Services Agreement (VYSA), or receive Ready, Set, Go (RSG) support). Youth are matched with a Youth-in-Transition worker who helps them build stability, strengthen life skills, and stay connected to the resources they need.

Youth-in-Transition Workers are system navigators who support youth with:

• One-on-one guidance around education, employment, housing, and goal setting
• Help navigating community services and supports
• Life skills development including budgeting, communication, and problem-solving
• Advocacy to make sure youth receive appropriate services from various organizations
• Workshops focused on skill-building, wellbeing, and planning for the future

This program is voluntary, youth-driven, and designed to help young people build independence at their own pace.

Referrals
To refer a youth to the program, please complete our referral form. Once submitted, a member of the Youth-in-Transition team will reach out to the youth and the referring worker to set up an intake and discuss next steps.

If you are outside of the London and Middlesex area, the YIT Worker Map can help youth, community supports, lawyers, child welfare workers, and others connect with their local Youth-in-Transition Worker.

YOU | Strathroy

Find us right in downtown Strathroy at 32 Front Street West. We’re open Monday to Friday, 9am – 4:30pm, with friendly staff ready to support you. This is a space for youth in Middlesex County, where you can connect to employment and housing services, get help, or just drop in when you need something. And if you can’t make it here in person, call us and we’ll connect with you right away.

  • Help with resumes, practice for interviews, interview clothing, and career planning
  • Free access to computers and phones for job and housing searches
  • Support finding safe and affordable housing
  • Hygiene products and other basic needs essentials
  • Emergency food bank
  • Access to community programs, including counselling for children and youth
