Reporting to the Community Outreach Team Leader, the Rapid-Rehousing Stability Worker (RRH program) will work on an outreach basis to support youth who are identified by the City of London’s Coordinated Access as needing support and assistance to obtain and maintain permanent housing. Rapid Rehousing is a housing-led intervention that supports individuals and families to quickly exit homelessness and return to housing in their community. At YOU, Rapid Rehousing uses a light-touch approach and is best suited for youth with stronger protective factors who do not require more intensive interventions. Participants are supported to secure housing, access rent supplements, coordinate move-in supports, and engage in ongoing case management. While many Rapid Rehousing programs are short-term, YOU’s model recognizes the unique realities of youth homelessness and offers longer-term, developmentally appropriate support.
Specific responsibilities include but are not limited to the following:
Build trusting relationships through consistent, collaborative, and goal-oriented interactions
Complete intake assessments and develop collaborative housing stability plans
Supporting youth in securing and maintaining housing
Facilitate living-skill development (budgeting, cooking, cleaning, safety planning)
Facilitate soft skill development (critical thinking, problem solving, self-awareness, Responsibility)
Provide referrals to crisis intervention services and safety planning, where appropriate
Advocate within systems (landlords, income supports, education, courts, health)
Collaborating with participants to build a support network consisting of natural and formal supports (mental health, addictions, cultural support, medical care)
Collaborate with key community partners (St. Leonard’s Community Services, City of London, Ontario Works etc.)
Track progress, document case notes, and update service plans
Uphold harm-reduction, trauma-informed, and culturally safe approaches.
Qualifications:
Completion of post-secondary education at a college or university level in a human relations or social services field or combined equivalent education and a minimum of 2 years related case management experience
Extensive knowledge of local support for youth
Knowledge of the Residential Tenancies Act
Experience working with individuals experiencing homelessness, mental health, trauma, and addictions
Current Police Vulnerable Sector Check
Current First Aid & CPR Certificate Level C
Experience working with at-risk youth and extensive knowledge of local community-based programs
Proficiency in working with Housing First approaches, specific to providing in-home support
Proven ability to manage multiple tasks, meeting targets and deadlines
Proficiency in Microsoft Office 360 including SharePoint, Word, Outlook, Excel, and PowerPoint
Must have access to reliable transportation .
Hours
35 hours per week, flexible scheduling depending on participant needs
Daytime, evening, weekend on a shift rotation.
Note: this position is expected to work on rotating shifts to ensure 7 days a week support to participants in the Housing First for Youth programs.
