Reporting to the Manager of Shelter Services, the Primary Case Worker will be responsible for overseeing case management plans. Primary Workers will be responsible for the successful day-to-day operations of the Youth Shelter, including conducting intakes/discharges, assessments, case management plans, coordinating referrals, and providing individual supports to youth accessing our shelter. Primary Workers will utilize a harm reduction and trauma-informed approach, exercising compassion and de-escalation strategies to support youth experiencing crisis, mental health imbalance, and behavioural challenges.
Responsibilities, but not limited to the list below:
- Administering assessments to youth (e.g. VI-SPDAT or Youth SPDAT) and initiating participants’ plans as soon as possible (e.g. diversion, family reconnection, rapid rehousing, or shelter intake, etc).
- Provide appropriate community referrals and supports to participants
- Complete participant case notes, occurrence reports, shift change forms in a timely manner
- Input on-line case information that supports the coordinated attainment of successful housing outcomes for youth in a shared case management approach within the broader context of homeless prevention in London
- Receive, record sort and store donations from the community
- Perform interior and exterior safety checks
- Utilize successful de-escalation strategies to support youth experiencing trauma, mental health, or behavioural challenges in accordance with YOU policies and procedures.
- Perform general cleaning duties
Qualifications:
- Completion of post-secondary education at a college or university level in a human relations or social services field, or combined equivalent education and experience
- Ability to work shifts: days, evenings, and weekends on a 5-week shift rotation.
- Current Police Vulnerable Sector Check (or can obtain upon hire)
- Current Standard First Aid & CPR Certificate Level C
- Experience working with at-risk youth and extensive knowledge of local community-based programs
- Proficiency working with the Housing First approach in an emergency shelter is preferred
- Proven ability to problem-solve and manage conflict
- Demonstrated strong written and verbal communication skills
- Proficiency in Microsoft Office 365 including SharePoint, Word, Outlook, Excel and PowerPoint
- Access to a vehicle or reliable transportation
